Temporomandibular Joint Disorders (TMJ / TMD) Treatment

TMJ / TMD disorders are functional problems affecting the temporomandibular joint (TMJ)—the joint connecting your lower jaw to the skull—and the muscles that control jaw movement. The TMJ is a highly complex structure and plays a key role in everyday functions such as speaking, chewing, swallowing, and yawning.Your jaw muscles and the TMJ work in perfect coordination. Even a small imbalance may lead to jaw pain, limited movement, clicking sounds, and discomfort that can spread to the face and neck.

Why Is the Jaw Joint So Sensitive?

The TMJ is closely linked to bones, muscles, ligaments, the joint disc, and your bite (how your teeth come together). That’s why TMJ problems may affect not only the jaw area, but also:
  • Head and neck muscles
  • Teeth and bite
  • Cheeks, lips, and salivary glands
  • The area around the ears, nose, and eyes
As a result, TMJ disorders may sometimes present as ear pain, headaches, neck stiffness, or dizziness.

Common Causes of TMJ / TMD Disorders

TMJ disorders are rarely caused by just one factor. The most common causes include:
  • Teeth clenching or grinding (bruxism)
  • Habits such as lip biting, nail biting, or frequent gum chewing
  • Chewing on one side only
  • Poor posture
  • Overuse or fatigue of jaw muscles
  • Long dental procedures (jaw open for extended periods)
  • Trauma, accidents, or injuries
  • Keeping the mouth open for a long time during general anaesthesia
  • Bite imbalance, misalignment, or crowding
  • Some systemic or psychological factors

Symptoms of TMJ / TMD Disorders

TMJ problems may cause a wide range of symptoms, including:
  • Pain or fatigue in the jaw and cheek muscles
  • Pain that radiates to the ear, neck, teeth, eyes, or nose
  • Tinnitus (ringing), dizziness, or general fatigue
  • Clicking, popping, or grinding sounds when opening the mouth
  • Limited mouth opening or jaw deviation while opening
  • Pain during yawning or eating
  • Difficulty biting or tearing food
  • A feeling that your bite has changed
  • Wear, sensitivity, cracks, or looseness in teeth
Important: Symptoms may fluctuate, but without treatment, there is a high risk of becoming chronic.

TMJ Examination and Diagnosis

A TMJ assessment usually takes around 30–35 minutes and requires a detailed evaluation. Your dentist may:
  • Review your medical and dental history
  • Assess symptom duration and intensity
  • Examine muscles and joints by palpation
  • Measure jaw movement range
  • Listen to joint sounds when needed
If required, additional imaging may be recommended, such as:
  • Panoramic X-ray
  • Dental CT scan
  • MRI (for disc and soft tissue evaluation)
In some cases, bite analysis, impressions, or model measurements may be used for a more precise diagnosis.

The main goals of treatment are to:
  • Reduce pressure inside the jaw joint
  • Restore muscle and joint balance
  • Control pain
  • Rebuild normal jaw function

  • Treating underlying dental issues
  • Night guard / occlusal splint therapy
  • Medication support (pain relief / muscle relaxants when appropriate)
  • Jaw exercises and patient education
  • In disc-related cases: arthrocentesis (joint lavage / injection)
  • Orthopaedic appliance therapy (when indicated)

  • Maintain good oral hygiene
